Statins and glucose Are they worse than 1,000 mg Niacin?

ONE
Statin and glucose Meta-analysis 2017 link

"The present meta-analysis confirms and reinforces the evidence of a diabetogenic effect by statins utilization. These observations confirm the need of a rigorous monitoring of patients taking statins, in particular pre-diabetic patients or patients presenting with established risk factors for diabetes."

TWO
Korean nation study of increased glucose with statins

Conclusions

"There was significant increase of fasting glucose in non-diabetic individuals in proportion to adherent and intensive use of statins.
Among the statin subtypes, use of
1-atorvastatin,
2-rosuvastatin,
3-pitavastatin and
4-simvastatin
were associated with significant increase in fasting glucose.
Pravastatin, lovastatin, and fluvastatin also had trend toward an increased fasting glucose, but were statistically non-significant.
These finding suggested medication class effect of all types of statins predisposing hyperglycemia although there was some difference in the degree according to the type. We need to find optimal strategy to maximize the cardiovascular benefit and minimalize the hyperglycemic effect by statins."

Low dose of generic statin, niacin and zetia to avoid side effects.

Adding 1,500 mg Niacin to statin and zetia increases risk of DM?

Effects of Extended-Release Niacin Added to Simvastatin/Ezetimibe on Glucose and Insulin Values in AIM-HIGH link
My solution:
Take only 1,000 mg of Endur-acin a day with 10 mg atorvastatin and half tab of Zetia.
If fasting glucose goes up greater than 95.
Start Low carb High fat diet or Atkins diet before stopping any meds.
Losing 5 lbs may correct the slight raise in glucose.
I have seen transient raises in glucose my patients on Endur-acin.  Probably related to weight gain? Not due to the low dose 1,000 mg of Nicotinic Acid.
If not at target lipid values increase statin slowly and take full tablet Zetia.

FIVE

High dose niacin is almost impossible to take.  That is why I gave my patients the proprietary wax matrix over-the-counter nicotinic acid at only1,000 mg a day.  With low dose statin and Zetia the triple treatment gives great results cheaply and few side effects.
Peter Attia misses the lessons of treating HTN, DM and HIV.  Multiple meds at low doses for synergy and safety.  He knows this from surgical oncology.
