I gain hope with this article (excerpt below)






 My diagnosis of Alzheimers was diagnosed in Dec 2017.
Now about 4.5 years after I don't want to travel anymore.  
It's just too stressful for me.
Stress makes me feel depressed.

Ginger left for Greece on July 8, 2022
My wife will be home by 2 AM on Tues. July 26,  2022
 from cruise to Spain and Portugal.  

Total 16 days away.

Routine helps me do well on my own 
Keeps me content.
Ginger insists on having a housekeeper come twice a week.
She is also paying for a geriatric homecare visit 2- 3 times a week just to check on me.
My routine:

Sleep (very important)
1-I sleep a with CPAP machine from 9 PM till 8 or 9 AM. 
I take some sleep supplements. 
2-Melatonin to keep sleep times consistent.
3-Ashwaganda link
I try to sleep till 9AM.

Daily Routine
Morning TV. I have an 85 inch tv screen.
I like Morning Joe.  I "tape" it.
It's 2 hours long and I speed through the commericals. 

The 'Smart' TV is a boone as well as a bane
The boone part is that the picture on the screen is wonderful. 
The bane part is when I misplace the remotes.

After watching about 3 hours of TV while looking at my computer writing twitters and blog entries I develop some mild anxiety.

I then swtich to entertainment TV.
I own many DVD's.
I am 8 seasons into the series VIKINGS.

Meals
I have coffee and diet pepsi to put off breakfast as long as possible.  I fill my 8 cup water pitcher which I have no trouble drinking it all because of all the pills I take.  
I take my prescription pills first thing in the morning.



I start to make a Salmon sandwich for breakfast/lunch.

My wife went away to Europe for a month. I did okay with the executive functions I still have.

 My recent memory is bad but I can still get by for a short period with my wife gone to Europe.

I have maintained the following skills:

1- Take my medicine in the morning.

2- Monitor my BM's so I don't get constipated. I recently started stool softener and prunes.

3- Deal with my 2 TV remote controls. 

4- Sign on the internet to write blogs and twitters

5- End my AM fast with a Salom sandwich Salmon sandwich link

6- Order TV discs on Amazon .

7- Call friends and family.

8- My many supplements link

9- I don't drive anymore and I am running out of my favorite foods;

        Can of soup which only takes 4 minutes in microwave.

oi        Premade packages of Chopped salad

       Fry hot dogs

I am satisfied with these meals.  I don't drink alcohol anymore.

8- Despite taking my Ozempic shot each week, I stll feel hungery so I have a snack every hour.

           Pear

          Plum

          Pistachios

         Yogurt

        Premium protein drink each day

I finally got my glucose under control by stopping ice cream

Why Am I exhausted today?

 I had a great night of sleep.

I awoke at 10 AM.  

We went to the bagel shop.  One large cup of coffee did not wake me up.  

We went shopping at Sams.  I walk slow. I don't get out of breath. 

I am back on all my supplements.

After being off them for a month I feel no differnce.  supplements link

Patients would come to me complaining of fatique.  After I ruled out anemia and thyroid problem I 
would start Prozac.  The patient usually responded withib 5 days and stayoed on the antidepressant.
The most important thing for a depression is exercise but that is the last thing the patient wants to do.  
I do go up and down a flight of stairs 10 times a days.  I am also on citalopam.  

I quess it can chalk it up to my Alzheimers.  A bad day.  I haven't had any arguments either.

In my 5th year having Alzheimer's my attempt to achieve contentment each day

Best thing is to have 8-10 hours sleep after taking:
1- Magnesium L Treonate 4,000 mg Link
2- Melatonin 5 mg
3- Ashwagandha link

Key is to avoid stress which causes me to feel bad with subsequent anxiety and depression. 
Arguments with my wife is a main cause of this.

Each moring after taking all my prescription medications(plus a stool softener) I get a cup of coffee with 90% dark chocolate to get my flavinoids.  
I also drink a can of diet coke.  This seems to help me have a bowel movement.  
I drink these while in my man cave in the basement watching morning JOE on my 85 inch TV.
I also now eat 2 piece of dried prunes.  
Despite talking probiotics each day I find this helps me have a BM. 
Again having a BM helps me with my contentment. 

I also am on my computer while watching TV. 
I was advised by someone at memory center to not do TV and computers.  
I get great satifaction by writing my blogs and twitters. 

I try not to eat till noon. 
Right now despite that 14 hour fast my glucose only dropped 4 points, 212 to 208. 
I take Invokana. 
It doesn't seem to work as well as before.
I quess I  shouldn't take cream in my coffee?
I will have to stop all carbs especially anything with cream in it. 

With my on twitter and blog I usually get tired and switch from news to something entertaining on TV.  
Right now I am watching Better Call Saul series. 

At night I am rereading the Richard Sharpe series by Bernard Corwall.  I often remember some of the passages.  Other than the enjoyment I hope I am refiring the neurons that I forgot. 

I am not doing the walking that I should.  That is a big lapse in my program,  if I get axious I will pace in the house which helps. 

I was off my supplements for a month on the trip to Ireland.  This week I started all of them again.  Hard to determine if they do any good for me?


I don't usually call people. I get tired after long phone conversations. 

I have a Men's group on Zoom everyweek.  I always enjoy that.  We usually discuss a book that we have chosen to read. 12 men from my Episcopal church.

I mention true grit in the title of my last book.

 
This quote may explain why I have gone publc with my diagnosis. 

Many people hide their diagnosis of cancer because there is often a stigma atttached to it. 

The same is true of Alxheimers.

I am writing the fifth year of my experience with Alxheimers to shine a light on what is the progress of the disease.

Many Docs are against this type of pill burden. 

Pill burden is a concept of the past since our experience with treating HIV successfully. 

I stopped supplements on my last trip last month.

Did I notice any problem? HARD TO TELL.

I am felling better back  home at with less stress of travel.

Update on my Alzheimers

I had some good glucose fasting numbers


I was diagnosed with Alzheimers in Dec. 2017.

It is now Feb 11, 2022. 

I am in my fifth year since the diagnosis was made.

I have kept to the plan of treatment quite well.

In fact, in Florida since Jan. 18 I have increased my walking and weight lifting. I seem to get tired easily. 

My weight stays the same 188 to 190 lbs. 

I skip breakfast and have good ketone levels.  

Fasting glucose is between 150 to 200.  

I rarely have a good glucose level. link


Diet, exercise, medications, supplements and sleep are doing well. I have vivid dreams which I enjoy.

I also keep up with listening to music and doing Duo Lingo in Spanish every day.

Anxiety is often a problem by noon. 

My Manhattan anchor helps but I recently subsituted my second cup of coffee with a soothing tea.

Exercise helps. 

The worse thing that can happen to my on any given day is to get into an argument with my wife. 

The stress from the distress of the anger link causes depression. 

My wife and I have done much better at not engaging in anger.  


I do feem my short term memory is worse and I was hoping adding on 10 mg of Aricept would help.

The RX cost $1400.

I am waiting for the generic version now. 

Hence I am always tintering with my plan. 

I am afraid to stop all my supplements. 

I continue with testosterone and Cialis link. 

It's one of the few drugs that I can actually see the good effect it has on me. 

Ozempic is another weekly injection that clearly curbs my appetite. 

I also see positive lab results with statin, fish oil and nicotinic acid.  

I think Invokana has saved my life by lowering my glucose. 

I avoid falling off the cliff with the anticoagulant, Xarelto (I have Atrial Fibrillation) 

I also keep my systolic blood pressure less that 120 with ramipril and a calcium blocker.

Winter vacation in Avon, Colorado in my 5th year of Alzheimers

I have had a great time this last week visiting my two sons with their wives and one grandson. 

My wife and I had a difficult time driving here in the snow: 

Stressful drive up the mountain link.

We arrived late at night and just before the main highway was closed.

However the next morning there was beautiful sunshine:

Now we are planning to drive home tomorrow.  We will probably not come back unless someone else drives us.

The week was wonderful we have only had flurries of snow. 

My kids tried to ski everyday.  They loved it. 

My wife would babysit the baby while they went out. 

I love my grandson but the last two mornings I stayed by myself for the quiet and calmness as I was the news and write my twitters and blogs. 

Looking forward to going home to my Smart TV and the recordings of Morning Joe, as well as the DVD player. 

I have filled some of the down time re-reading Bernard Cornwall series on the Penisula War of Britain vs. Napolean in the Richard Sharpe series. 

I am reading Sharpe's Gold for the third time.

I first read it 10 years ago.

I take my time and underline more.  

It has been a great joy to rediscover these books.

We save money by not eating out. 

Ginger and I brought a great deal of groceries from Topeka. 

She cooked six thick pieces of Salmon two nights ago. 

My son Andrew made a great plate of Shrimp Curry.

Of course there was wine and red wine. 

I also make my usual anchor Manhattan each day.

However it has been very cold outside.  I get cold so easily.

I did walk over to the liquour store a few blocks away, yet 

the sidewalk was still covered with some snow and ice. 

Thanks goodness I had my snowboots on.  

My kids have used the outdoor hot tub everyday.  

I forgot my swimming trunks but I am not sure I would want to go out into the cold.

There is a lift to Beaver Village that is free and I would have liked to visit again.  Again even though the temperature is up to 36 degrees F.  I decided not to go. 

So here I am by myself listening to MSNBC talk only about the Jan 6 insurrection that happened 1 year ago while so far I haven't seen Fox news talk about it at all. 

I am content.  We had a free breakfast that was included with our points. I am warm and am grateful I can still write these blogs which will make up the YEAR 5 book that I will title Trying to Avoid Falling off Cliffs. Subtitle Doing Better than Expected

The main goal for me is to maintain to maintain my routine .

1- Medications: Ten different types in AM

2- Supplements: Twelve different types after lunch.  

I have not been checking my fasting glucose, ketones or weight. 

3- Exercise: I am able to walk 4,000 to 8,000 steps by pacing in my room each day.

I had abundant amyloid plaque on my Pet scan but my neurologist gave me the diagnosis of Alzheimers based on having one gene for AZ and serial cognitive exams.

Support for supplements for Alzheimers

This my edited copy of this report from Medscape. 

 "Investigators found a baseline history of CVD, including transient ischemic attack, congestive heart failure, coronary artery bypass graft, percutaneous transluminal coronary angioplasty, and stent, 

but not myocardial infarction or stroke as these were excluded in the parent trial because they affected the response to multivitamins.

As expected, those with CVD had lower cognitive scores at baseline. "But after an initial bump due to practice effect, at year 1, the cardiovascular disease history folks continue to benefit from multivitamins, whereas those who got placebo multivitamins continue to decline over time," said Baker.

Based on information from a baseline scatter plot of cognitive function scores by age, the study's modeling estimated the multivitamin treatment effect had a positive benefit of .028 standard deviations (SD) per year.

"Daily multivitamin-mineral supplementation appears to slow cognitive aging by 60% or by 1.8 years," Baker added.

To date, the effect of MVM supplementation on cognition has been tested in only one large randomized clinical trial — the Physicians Health Study II. That study did not show an effect, but included only older male physicians — and cognitive testing began 2.5 years after randomization, said Baker.

"Our study provides new evidence that daily multivitamin supplementation may benefit cognitive function in older women and men, and the multivitamin effects may be more pronounced in participants with cardiovascular disease."

For effects of multivitamins on Alzheimer's disease prevalence and progression, "stay tuned," Baker concluded.

Following the presentation, session co-chair Suzanne Schindler, MD, PhD, instructor, Department of Neurology, Washington University School of Medicine, St. Louis, Missouri, said she and her colleagues "always check vitamin B12 levels" in patients with memory and cognitive difficulties and wondered if study subjects with a low level or deficiency of vitamin B12 benefited from the intervention.

"We are asking ourselves that as well," said Baker.

"Some of this is a work in progress," Baker added. "We still need to look at that more in-depth to understand whether it might be a mechanism for improvement. I think the results are still out on that topic."

The study received support from the NIH/NIA. Pfizer Consumer Healthcare (now GSK Consumer Healthcare) provided study pills and packaging. Baker has disclosed no relevant financial relationships.

Pill burden for Alzeimers?

Chapter Two

 I take 39 pills a day.

Also a testosterone shot and an ozempic shot once a week.

When I was in my residency I was taught there is a limit how many pills you can take before there is a problem with interaction. 

There was also called 'pill burder' which decreased compliance. 

This is why my YEAR 4 book (now at pulblishers) talks about the need for True Grit and Fortitude with Alzheimers. 

When I did my Infectious disease fellowship with the advent of HIV (AIDS) many of the medicine myths were crushed.

Pneumocystis carinii pneumonia (PCP) is an opportunistic infection that occurs in immunosuppressed populations, primarily patients with advanced human immunodeficiency virus infection.

PCP must be treated with prescription medicine. Without treatment, PCP can cause death. The most common form of treatment is trimethoprim/sulfamethoxazole (TMP/SMX), which is also known as co-trimoxazole and by several different brand names, including Bactrim, Septra, and Cotrim.

We treated this with very long term treatment. 

I also learned we would treat Staph aureus chronic osteomyletis for years with an oral cephalosporin.  

Thus I accepted doubling the number of pills I take under the guidence of a Holistic doctor who was familiar with the Bredesen ReCode treatment for Alzheimers.

 Introduction to Year Five

Avoiding falling off Cliffs with Alzheimers

Am I one of the Alzheimers survivors?

I guess I was too pessimistic when I wrote my Year One book.  I am waiting for when I forget I have Alzheimers. I thought with Namzaric I could only hope for 2 good years before I fell off the cliff of the survival curve and had a severe loss of cognition.  Afterall I don’t do a great job controlling my Diabetes Mellitus type 2. 

Other than the head injury which I had with a small subdural hematoma when I fell after drinking I doubt I will fall off a sudden cliff because my blood pressure is low and my cholesterol is very low with medications.  Thus no stroke or heart attack is in my future. 

During Covid shutdown I decided to try supplements as advised by Dr Bredesen and Dr Isaacson. They both had poster children for their program.  Julie and Rogen.  Thus I started the ReCode program at a Holistic clinic in Lawrence, Kansas.  I have completed twelve.   months and don’t plan to continue with visiting the clinic now that my year's contract is done. I mostly wanted a Doctor familiar with all the supplements to slowly guide me on starting them.  Presently I take  fourteen new supplements.  I tolerate them well except for some mild esophago-gastric reflux.  Have they helped me? All I can say is I seem to be maintaining the status quo on the most part.  My wife and I still notice some more short term memory loss.  I do think I am more emotionally stable which I contribute to Ashwagandha. 

What will the Fifth Year bring?

I am open to taking the new Alzheimer's drug, Aduhelm (aducanumab).

I am waiting for the one year post treatment analysis before I make a decision. 


In the meantime I plan to have fun. 

We have scheduled 6 cruises, one Tauck tour in Ireland and 3 USA trips. 

After next year I am willing to roll the dice on Aduhelm (aducanumab) and get the expensive monthly  IV infusions each month.

Third review of On Pluto

 I have read page 341.

Last pg is 364. 

Time to write the Third review of 

On Pluto by Greg O'Brien.

This is a good book. 

I was a little critical at first because of his use of stories from other sources to be funny or illustrate his points.  Most of which I have heard before.

Probably I am jealous of all his contacts.  

He even name drops people from his childhood in Rye, NY. 

This book is long because it is an autobiography. 

Again his life is much more interesting than mine. 

I don't have one tenth of his friends. 

I know he is making a context which is fine. 

The chapter that won me over was the one written by his wife, Mary Catherine McGeorge O'Brien. 

Chapter 24 page 329 to 341.

This chapter is amazing and I hope my wife will read it. 

"Ironically, Alzheimers now is starting to heal our marriage."

Hopeful words for my wife and I.

                          

In chapter 23,  Greg O'brien puts his investigative reporting skills in action describing how 12 different Alzheimer patients he knows dealt with their Alzheimers.  Very well written and the title, "which one of you nuts has any guts".   A quote taken from One Flew over the Cuckoo's nest" but very apt. 


I love his historical references because some of them I was not well acquainted with.

Sports references are likewise fun for me and because I heard them for the first.

Popular TV and Movie culture not so much as they were very familiar to me as well as his jokes that I have heard before. 

I quess I am being a tough critic.  Again probably jealousy as he has gotten so much attention. 

When I finish the book soon I will write a book review for Amazon.

I will say this book as done a great service for me. 

It is the only book written in the first person that informs me what to expect as I advance from Early Alzheimers to Moderate Alzheimers. 

The hopeful news is similar to him I will be able to write and talk.  

One thing I will do different I will take stronger drugs for depression if needed. I am doing well with Citalopam 40 mg.  He only takes 20 mg.

If I had his level of rage I would definitely take stronger drugs.  I know I would not want to put my wife through that and she would not tolerate it.  I would be in a nursing home. 


I have more rage than ever but I have never acted out as he has.  

I also listen to the people around me telling me what to do.  

As his wife says, he is an asshole. No humor or jealousy involved.

I gave up my lisence to practice medicine and decreased my driving. 

He continued working and puts himself in impossible situtations which cause him great stress. 

I try to avoid stress.  

Stress is not good for Alzheimers. 

Distress causes decreased judgement, anxiety and depression. 

I will have a cocktail after an argument with my wife to relieve the stress. 

Ashwagandha has helped me as well. 

He says he is taking supplements but he doesn't say which ones other than what Dr Rudolph Tanzi advises.  






Review of The First Survivors of Alzheimers by Dr Dale Bredensen.

 I preordered this new Bredesen book. 

It finally arrived yesterday.

This is the first book to answer the question I asked in a blog in June 2021:

I started the ReCode protocol after reading about Julie G.

With the six survivors discussed in this book and the new AZ drug that seems to be questionable at this point I feel validated in my decision to start the program at Atma Holistic Clinic in Lawrence, KS.  


Also after 10 months on the program I am tolerating all the supplements and cost link. 


I also feel better.  I did well, I think on my extensive three hour yearly cognitive this week for the non-treatement trial I am in.  My fourth MRI result link

of Alzheimers Disease by Richard Isaacson  and Christopher Ochner

The following supplements are advised:

B vitamins

Vitamin D3

Omega 3 fatty acids

Cocoa powder

Curcumin

Resveratrol


They may need an update as in the End of AZ by Dr Dale Bredesen they leave out many other options and don't mentione. 

The ReCode protocol by Bredesen does include all the above but Ochner does not advise Gingo Balboa or Co-enzyme Q.

Second review of On Pluto

Second review of on On Pluto by Greg O'Brien.

This time I will post what I think Greg O'Brien got right. 

1- Exercise. 

Author says it recharges his brain.

He does a great deal of running. 

I suspect it's one of the main reasons he is hanging on with Moderate severe Alzheimers. 

He does seem to pay a price with subsequent body pain.  Pain causes distress and this is not a friend to the AZ brain. 


2- Medications p23

This is the only AZ book that at the beginning of the book discusses the usual medications for AZ.

Aricept

Namenda

Trazadone to help him sleep

Celexa (citalopram) 20 mg:

 An antidepressant for his rage, which considering how bad his rage I can't believe he is not on 40 mg which I take. 


3- Socialization

Mr. O'brien has a lot of friends that he stays in touch with and gets good support.

9/ Great discipline with memory helpers on his phone and gmail. 

10/ Author writes 2 hours every night. 

I also try to write in my blog and twitter everyday. 

I feel great gratification when I can write a good thread on twitter or a good blog.  I encorporate my blogs into my last 3 books on my AZ which I document in diary form. I am now working on my Year 4 book.  

The author and the author of Tattoo on my Brian both taught me about cognitive reserve which will hopefully allow me to continue my writing even as I enter moderate alzheimers.

The cheapest easiest way to TRY to prevent progression of Alzheimers.

 Most people can't afford $7,000 to enter a Dr Dale Bredensen's ReCode program. 

Also the supplements can cost $1300 a year. 

Most people don't want to take so many pills. 


So what does Medicare pay for?

1- Namzaric

2- Invokana for diabetes

3- Ozempic shots for DM and weight loss

4- Metformin for DM

5- Blood pressure pills 

6- Statins, Zetia.

7- Testosterone

8- Citalopram

Ask your Doctor which are good for you. 

In my opinion these are the pills that will prevent you from suddenly falling off the cliff with Alzheimers by a stroke, heart attack.

Atkins or Low Carb High Fat diet to get into daily Nutritional Ketosis

This is a very individualized item.

The elderly need to be careful to avoid injury. 

Walking 20 minutes a day is the easiest path. 

Then if anxious do pacing in the house to get up to 10,000 steps. 

If you can do more with weights, good.  I mayself have cut back on weights due to some joint pain but I try to do curls with light weights.  

Some advise HIIT ( high intensity interval  training).  I think this is fine for a younger group, not practical for eldlerly.  

My HIIT is going up and down the basement stair 10 times a day. 

It's probably the most dangerous thing I do all day.

My Florida neurologist was worried about my falling and told me to walk in the pool 30 minutes three times a week.  I did it once.  After doing ten minutes I later realized I had hurt my right hip which still seems tight at time months later.

A new fun experience with my wife

 


My daughter and her husband purchased a Saloonbox kit for me for Father's Day.  

I like mixology but the directions seemed complicated to me so I asked my wife to help me.

I have a home bar which I call The See No Evil Bar link

I make a great Manhattan each day.  That is 3 ounces of bourbon a day. 

I am doing the Recode protocol of Dr Dale Bredesen for Alzheimers. 

He allows some wine.  Four ounces of wine is only one ounce of alchol. 

I have a nutritional 'therapist' with Atma clinic link

I confessed to her that I have a cocktail each day, either a Manhattan or a Vodka martini.  

She gave me absolution. 

She said it 'anchors' my day.

It really does.  It's a mental activity that I enjoy.  I put off breakfast till 11AM and then I put off lunch with a Two PM Happy Hour. I have a small bag of pistachios with the drink. I often start a movie as well. 

This Saloonbox Strawberry mix took it a notch higher.  More ingredients.

I don't care for sweet drinks anymore but this was good. 

I had my wife check me on each step of the directions which was fun.

Fathers day was a while ago so it took me a while to do this new challenge. 

My wife's willing participation was what got me out of my apathy to try something new. 


I have read you should not drink 5 ounces or more of alcohol a day.  

With 2 glasses of Red wine I usually exceed that amount. 

Except for my subdural hematoma link, I have not had any medical problems with drinking. 

I am more careful now by drinking water between alcohol and waiting a while. 

I love to buy the beverage package on cruises.  I never went to bars when I was young but now I am a barfly on the cruise as it is great socialization meeting new people.  I often have 10 alcoholic beverages during the day. 

YOU CAN'T DRINK ALL DAY, if you don't start early in the morning.

I always start the day on the ocean at the baristic with an alcoholic coffee.  Then wine at lunch and dinner.  I have a cocktail at the bar before dinner and sometimes stop at the bar before bed. 

Most people reading this must go "tsk, tsk".

1- Too much alcohol?

2- My lipid panel levels  link on statin are too low? LDLc 31

3- I have been on Atkins since 2011 and in nutritional ketosis.

4- My systolic BP is usually between 90 and 120.


My reply is I have done better than expected with my AZ diagnosed in Dec 2017.

I am down to my lowest weight since my marriage 14 years ago. 200 pounds. 

I take Ozempic 1.0 mg a week which has helped with the last 10 pound weight loss. 

Depression, anxiety and apathy are a big problem with Alzheimers.

People with chronic diseases such as obesity, diabetes and AZ need to find a way to deal with it as they are the ones who with True Grit and Fortitude must follow a regimen of multiple medications, supplements, diet and exercise to try and prevent the sudden falling off of a cliff that can occur with AZ due to stroke and heart attack.

I avoid hopelessness and apathy each with my cocktail 'anchor' each day.  

I am writing my fourth year book of how I have tried to enjoy my life despite the cloud of the inevitable fatal Alzheimers hanging over my head. 

Each year that I finish a book I feel a great deal of accomplishment. 

I am trying to give people first person documentation of how I am doing it with the goal that they will make their diagnosis early and realize with Mild cognitive impairment you have several years of life ahead of you.
